or advise you. What is a Home Study ?
A home study is merely a report required by adoption officials
affirming that you are suitable to be a parent(s) of a child and
can provide the stability and home environment that a child
needs. It consists of possibly 2 interviews and a home visit.
You will likely be asked to write a brief autobiography and to
submit the following photocopies (not originals):
- Preparation: Write an autobiography: the agency you choose
may or may not require this step.
- The following copies may be needed during the home study.
- Photocopies of Birth Certificates for each family member.
- Photocopy of Marriage Certificate.
- Photocopy of letter from local police stating "no
criminal record".
- Photocopy of financial statement.
- Photocopy of medical from your doctor on his/her letterhead
stating positive condition of physical/mental health.
- Reference letters
- Post-Placement agreement must be in the home study.
All of the above may be required from the agency to do
your home study.

Because
you are adopting from abroad, you need to get their approval for
a visa for the child.
- If you do not have the forms for an adoption packet
application you may either:
- Call the INS for the forms. 1-800-870-FORMS
- Download from the internet by clicking : http://www.ins.usdoj.gov/graphics/formsfee/forms/index.htm
- Fill out the I-600A only.
- Send photocopy of Birth certificates, or proof of
citizenship.
- Send photocopy of divorce decree if applicable.
- Send photocopy of marriage certificate.
- Favorably recommended home study.
- $460 plus $50 per person for fingerprints
All
of the below documents must be originals. Dossier Check-List
- Send application to Children's Hope now and begin to collect
all documents. Our Adoption Guide and case workers will give you
clear direction for the following:
- General Power of Attorney (Form I-1)
- Home study/Agency License and Post-Placement agreement
- Declaration of Willingness (Form I-2)
- Additional Declaration (Form I-3)
- Birth Certificates
- Marriage Certificate
- Medical Certificates-each parent.(I-4)
- Infertility letter from physician
- Medical Release Form (I-5)
- Bond (I-6)
- Statement of Financial Assets (I-7)
- Employment letters
- Income Tax Return
- Police Clearance
- Collect the documents with the assistance of your CHI
personal Adoption Consultant.
- Get them notarized. All instructions are in the
Adoption Guide.
- Get them state certified. Direct access to
information on CHI Family NetŪ
- Send them to Children's Hope for processing. Personal
/ medical information and a picture of your child will be sent
to you for your approval.
