As a licensed, not for-profit, tax-exempt agency with a mission to children, your gift is deductible to the full extent allowable by law. You will receive a receipt for your contribution. Please fill in the spaces below and indicate your area of interest and commitment. 

            [    ] Medical Vision Clinic (Ophthalmologic) in India.  Can you commit $100
                    or more.  Our goal is $160,000 away and we want to complete in 2002.

[   ] Sponsor school tuition @ $60.00 per year.

                   (each $250 donated will be matched by CHI for the $500 per child

[    ] Support one of the 120 teachers giving free education to the poor in India:
                    Only $32.00 per month

[    ] Dig a well for a community in India:  $450.00  Named in honor of donor or in the name
                   of someone you wish to honor.

[    ] Provide a job building a Pedi cab which will be given to an unemployed father
                    in our work in India:  $200
