          In January 2002, the US Consular Service introduced a new procedure for Russian citizens seeking non-immigrant entry visas to the United States.  This procedure requires citizens to provide supplementary information regarding their military service, participation in the military conflicts, their college or university degree, a list of countries visited in the last 10 years.

 

          Please be advised that as of May 01, 2004, US citizens seeking a Russian entry visa are required to provide similar information too.  Thank you for understanding.

 

          Please complete one new visa application for each person traveling to Russia.  DO NOT COMPLETE  #8, 9, 10, 11, 12, 13, 15, 16 and 39.  We will complete these blanks here at CHI. 

 

          If you would prefer filling this form out on your computer, you may download it from the Russian Embassy website.  That address is www.russianembassy.org.  Click on tourist visa and you will be able to download this document in Word format.  Be sure to sign the application before mailing it to us.

 

          DO NOT ATTACH THE PHOTO TO THE VISA FORM.
